"ShihTzu's by Connie".
Print this page, read it, sign it, date it and send it in with your Deposit, "ShihTzu's by Connie" will sign, date and return a copy to you.
All puppies sold will be honestly represented and evaluated as accurately as possible. Your puppy has been seen by a licensed Veterinarian several times prior to shipping/delivery.
A Health certificate from my vet will be issued at the time of delivery.
"ShihTzus by Connie" cannot guarantee the size, color, temperment or breeding ability of your puppy.
You will be provided with the shot/dewroming records of your puppy at the time of sale.
All puppies are considered healthy at the time of sale.

"ShihTzu's by Connie" guarantees this puppy to be free of Genetic Defects for a period of and not more than, "one year" from date of purchase. During the course of the "one year guarantee" your puppy must obtain all shots, dewormings, heartworm pevenitives, rabies and any medications recommended by your Vet. and administered by your Vet according to the puppy's age.
You must keep a record of such. THERE IS NOT A GUARANTEE AGAINST PARVO, CORONA, WORMS, COCCIDIOSIS or any know natural occuring puppie diseases, that are preventable by puppy Vaccine's.
You must have your puppy examined by a licensed Veterinarian within 72 hrs. after receiving your puppy, at buyer's expense.
If this puppy is considered a poor health risk in the opinion of your Veterinarian, we will provide you with a healthy replacement puppy, once these conditions are met.
The puppy must be returned within 24 hours after exaim, at buyers expense, along with any AKC papers, signed back over to us, followed by a Signed Statement from you Vet. stating why the puppy is a poor health risk.
In the event a death should occur, the puppy must be refrigerated, not frozen, until an autopsy can be preformed by your Vet., at buyer's expense. If the cause of death is due to Genetic, and NOT bacterial, or accidental death, a full refund, minus shipping and Vet bills, will be offered, once we have your Vet's results sent back to us, and looked over by my Vet.
This Guarantee applies towards your puppy only, and only as long as you own this puppy, contract is Voided if you sell this puppy within one year of purchase.
